Approval
A person deciding, recorded so that the decision can be checked later.
What it is
When policy answers REQUIRE_APPROVAL, an obligation is opened. It is discharged by a person reviewing exactly what will happen and signing that specific statement — not by clicking a button that sets a flag.
An approval satisfies the human-approval condition. It does not itself grant authority: the permit and the policy still have to allow the action.
